I think what you're suggesting about reporting stuff is a good idea and will contribute if I can. There could be a couple of reasons that people aren't submitting their incidents and I'm wondering what you think about the following.
Some people may not want to go through the hassle of a formal incident report whilst others may be too embarrassed. Would you be willing to receive details from people on the basis of protecting their anonymity if they prefer it that way? It could be that you can persuade them to change their mind but if not, isn't it better to get stuff in the open anonymously than hidden completely?
Also, as I'm a member of other clubs I have previously reported incidents via another club. If there's a useful lesson to learn should that be shared with you too, on an informal basis?
